RN - Clinical Care Navigator

Job description

Moab Regional Hospital is looking to fill one full-time position for an experienced RN to be responsible for directing patients to the appropriate route to receive care in the clinical setting. This person will be part of a team that focuses on patient navigation, assists with chronic disease case management, and provides patient education and assessment. The CCN will perform patient navigational duties via phone, email, or in person, with a strong focus on promoting a positive patient experience. Additional responsibilities include involvement in chronic care management and annual wellness visits in collaboration with physicians. Chronic care management includes assisting patients telephonically and in person, on a monthly basis, in managing the patient’s plan of care for chronic medical conditions to ensure quality of life is met and to avoid hospitalizations. Annual wellness visit responsibilities include providing nursing care for Medicare patients under the direction of a physician or health care provider.

Clinical Care Navigator Responsibilities:

  • Assist patients telephonically, using email, or in person to help patients understand available options to be seen by a provider.
  • Focuses on enhancing positive patient experience through strong customer service and communication skills.
  • Provide telephone education and advice when necessary and appropriate.
  • Deliver education and referrals if needed.
  • Take patients vitals such as blood pressure, temperature, and weight.

Job requirements

Minimum Qualifications

  • Current, unencumbered Utah or compact state RN license with minimum two (2) year’s RN experience.
  • Graduate of an accredited school of nursing.
  • Excellent customer service and communication skills including oral, written, and listening with providers, co-workers, patients, and patient families.
  • Knowledge of infection control principles, patient safety techniques, and prevention of adverse reactions to medications/treatments.
  • Demonstrated critical thinking skills and ability to maintain such in situations with rapidly changing circumstances.
  • High level of self-motivation and able to work well both independently and as part of a small team.
  • Must be reliable, punctual, and emotionally mature.
  • Ability to work in a confidential manner.
  • Must have a positive attitude. Knowledge of the principles and skills needed for practical nursing to provide patient care and treatment.
  • Knowledge of common safety hazards and precautions to establish a safe work environment.
  • Requires adherence to all policies and procedures, including but not limited to standards for safety, attendance, punctuality and personal appearance. As well as the MRH Employee Expectations as outlined in the employee handbook.
  • Must be flexible and willing to accept other responsibilities that may be assigned.
  • Comprehensive knowledge of a variety of pharmacological agents used in patient treatment, including dosage calculations.
  • Knowledge of computer systems and proficiency in use of basic word processing, data entry and electronic medical records.
  • High level of comfort with patient triage. Able to perform independently.
  • Schedule flexibility, including willingness and ability to regularly work evenings and occasional weekends and holidays.
